Asian Community Care Organization (ACCO) is seeking a dedicated and experienced Social Benefits/ Housing Specialist to support the organization’s Social Care Network (SCN) team and manage housing or Medicaid related cases for enhanced-service members. In this role, you will be instrumental in supporting high-need individuals with their social benefit application, Medicaid application, housing application, housing transitions, utility setups, and rental assistance. You will work closely with the SCN program leads to ensure accurate documentation, proper billing, and effective community support.
Key Responsibilities
- Manage cases involving Social Benefits Application, Housing Transition and Navigation, Pre-Tenancy, Rent/Temporary Housing, Utility Setup/Assistance, and Community Transitional Supports.
- Assist members with Medicaid Renewal application, Medicare Savings Program application, and New Medicaid Applications.
- Complete comprehensive housing assessments, collect required documentation, maintain accurate case notes, and coordinate effectively with members, landlords, hotels, utility companies, vendors, and community partners.
- Process and support requests for rent arrears, rental assistance, utility arrears, utility activation, furniture, moving expenses, security deposits, and temporary housing.
- Prepare invoices, receipts, proof of payment, and all necessary supporting documents for PHS submission.
- Track each member’s authorization, service caps, payment status, reimbursement status, and overall housing outcomes.
- Coordinate with Social Care Network Lead Entity during weekly or bi-weekly Housing Hub check-ins and accurately report on available organizational capacity.
- Maintain and continuously update an approved vendor and landlord contact list.
- Promptly escalate cases involving eviction, legal issues, domestic violence, fraud, or high-risk situations to management and appropriate external partners.
- Provide in-depth member education and follow-up in Chinese as needed.
- Assist with other SCN services (such as Screenings and Navigation) and support other ACCO-funded programs if needed.
Required & Preferred Qualifications
- Bachelor's degree in Social Work or a related field.
- 1-2 years experience in housing navigation, social services, case management, benefits administration, or community-based programs.
- Bilingual proficiency is required (English and Chinese).
- Strong administrative capabilities, including robust documentation, Microsoft Excel proficiency, invoice-tracking, and follow-up skills.
- Familiarity with NYC housing resources, landlord communication protocols, rental assistance, and utility programs is highly preferred.
- Comfortable, professional, and empathetic when working with high-need members and navigating sensitive housing situations.
- Familiarity with the Social Care Network (SCN) program, its framework, is a strong plus.
- Experience using SCN-related platforms, specifically Unite Us, is also a strong plus.
- This role will be assisting members from ACCO to ensure appropriate care is assessed by conducting in home visits.
Reporting Structure & Initial Scope
- This position reports directly to the Director of ACCO.
- The successful candidate will start with a controlled caseload. The scope of work will expand based on demonstrated documentation quality, adherence to payment controls, and available capacity.
Pay: $23.00 - $29.00 per hour
Work Location: This position covers all boroughs - 100% field work.
