Role Overview
PTs and PTAs provide skilled therapy services to patients in their homes, supporting recovery, mobility, safety, and independence. You’ll collaborate closely with the interdisciplinary team to ensure coordinated, high‑quality care.
Expanded Responsibilities
Physical Therapist (PT) Responsibilities
- Comprehensive Evaluations: Conduct initial assessments, SOC/Admissions, and reassessments using standardized tests and clinical judgment.
- Plan of Care Development: Create, update, and manage individualized treatment plans aligned with physician orders and patient goals.
- Skilled Interventions: Deliver therapeutic exercises, gait training, balance programs, neuromuscular re‑education, transfer training, and pain‑management modalities.
- Clinical Decision‑Making: Modify treatment plans based on patient progress, safety concerns, or changes in condition.
- Care Coordination: Communicate with physicians, case managers, nurses, and other disciplines to ensure seamless care.
- Patient & Caregiver Education: Teach home exercise programs, safety strategies, equipment use, and functional mobility techniques.
- Documentation: Complete accurate, timely visit notes, reassessments, and discharge summaries per agency and regulatory standards.
- Supervision: Provide required oversight for PTAs and Physical Therapy Aides, including 30‑day supervisory visits.
- Quality & Compliance: Participate in case conferences, in‑services, and performance‑improvement initiatives.
Physical Therapist Assistant (PTA) Responsibilities
- Treatment Implementation: Provide skilled therapy interventions under the direction and supervision of the PT, following the established Plan of Care.
- Functional Training: Assist patients with strengthening, mobility, gait, balance, endurance, and ADL‑related activities.
- Progress Monitoring: Observe and report patient responses, safety concerns, and changes in condition to the supervising PT.
- Home Exercise Instruction: Teach and reinforce home programs, safe mobility techniques, and caregiver participation.
- Documentation: Prepare accurate visit notes, progress updates, and care‑plan contributions in accordance with agency policy.
- Care Team Collaboration: Participate in interdisciplinary care planning, case conferences, and in‑service education.
- Patient Advocacy: Support patient independence, safety, and functional goals within the home environment.
- Compliance & Quality: Follow agency policies, regulatory requirements, and best‑practice standards for home‑health therapy.

Requirements
- Active California PT or PTA license
- Reliable transportation and willingness to travel within your region
- Prior home‑health experience preferred
- Strong communication, documentation, and patient‑care skills
Pay: From $75.00 per hour
