We are seeking a dependable, organized, and compassionate Home Care Coordinator to support our growing home care operations in the Amsterdam, NY area and surrounding communities.
The Home Care Coordinator serves as a key connection between our caregivers, patients/clients, families, and internal operations team. This position is responsible for coordinating caregiver schedules, maintaining consistent communication, resolving day-to-day staffing issues, and helping ensure that clients receive reliable, high-quality home care services.
The ideal candidate understands the fast-paced nature of home care, communicates well with caregivers and families, and is comfortable managing changing schedules and staffing needs.
Coordinate and maintain caregiver schedules for clients throughout the Amsterdam service area.
Match Home Health Aides (HHAs) and Personal Care Aides (PCAs) with clients based on availability, location, schedule, and client needs.
Fill open shifts, call-outs, and last-minute staffing needs promptly.
Confirm caregivers are aware of their schedules and assignments.
Monitor uncovered cases and work proactively to maintain continuity of care.
Coordinate schedule changes with caregivers, clients, families, and appropriate internal departments.
Assist with establishing coverage for newly opened cases.
Maintain regular communication with HHAs, PCAs, and other field staff.
Build positive relationships with caregivers to support engagement and retention.
Assist caregivers with scheduling questions and general employment-related concerns, directing issues to the appropriate department when necessary.
Follow up with caregivers regarding attendance, availability, call-outs, and schedule changes.
Identify caregivers interested in additional hours and match them with available cases.
Partner with Recruiting and HR to communicate current staffing needs in the Amsterdam area.
Communicate professionally and compassionately with clients and their families.
Respond to scheduling questions, concerns, and changes in a timely manner.
Notify clients and families of caregiver changes when appropriate.
Escalate clinical, service, or compliance concerns to the appropriate supervisor.
Help maintain strong client satisfaction and continuity of service.
Maintain accurate scheduling and caregiver information within the agency's home care management system.
Monitor caregiver attendance, missed visits, late arrivals, and other scheduling exceptions.
Assist with EVV-related follow-up and ensure scheduling information is accurate.
Document important caregiver and client communications according to agency procedures.
Work closely with Nursing, Recruiting, HR, Payroll, and Operations to resolve caregiver and client issues.
Follow agency policies and applicable New York State home care requirements.
Protect client and employee confidentiality and follow HIPAA requirements.
High school diploma or equivalent required; associate or bachelor's degree preferred.
Previous experience in home care, healthcare staffing, scheduling, customer service, or coordination strongly preferred.
Experience working with HHAs, PCAs, or home care clients is a plus.
Strong organizational and time-management skills.
Ability to manage multiple priorities in a fast-paced environment.
Strong telephone, written, and interpersonal communication skills.
Comfortable handling difficult conversations and resolving scheduling conflicts.
Basic computer proficiency and ability to learn home care scheduling and EVV systems.
Bilingual skills are a plus based on the needs of the local caregiver and client population.
