Schedule: Full-Time | Monday–Friday | 8:00 AM – 4:30 PM

Are you an experienced Licensed Practical Nurse (LPN) with a passion for delivering quality care in patients' homes We are seeking a dedicated Home Health LPN to provide skilled nursing care throughout West Philadelphia, serving patients in the 19104 and 19139 zip codes.
This is an excellent opportunity for an LPN with strong home health and wound care experience who enjoys working independently while making a meaningful difference in patients' lives.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN and physician.
- Perform patient assessments and report changes in condition to the care team.
- Deliver and document prescribed treatments and nursing interventions.
- Perform wound assessments, dressing changes, and ongoing wound care management.
- Administer medications and monitor patient responses.
- Educate patients and caregivers on medications, disease management, wound care, and home safety.
- Maintain accurate and timely documentation in the electronic medical record.
- Collaborate with nurses, physicians, therapists, and other healthcare professionals to coordinate patient care.
- Ensure compliance with agency policies and regulatory standards.
Qualifications
- Current Licensed Practical Nurse (LPN) license.
- Minimum 2 years of recent Home Health experience required.
- Strong wound care experience is required.
- Excellent assessment, communication, and organizational skills.
- Ability to work independently while managing daily patient visits.
- Valid driver's license, reliable transportation, and current automobile insurance required.
Schedule
- Monday–Friday
- 8:00 AM – 4:30 PM
- Full-time, 40 hours per week
- Travel throughout the 19104 and 19139 service areas.
