Job Description: This role is responsible for providing direct psychiatric rehabilitation, individual and group clinical services, and pre-employment and vocational supportive services to adult consumers via Personalized Recovery Oriented Services (PROS), Home and Community Based Services (HCBS) and/or Community Oriented Recovery and Empowerment (CORE).
Requirements:
PROS Specialists can be hired in both licensed and unlicensed capacities.
To be considered for a licensed PROS Specialist position you must possess a CASAC, Master’s degree in Social Work or a related Mental Health Discipline, a Registered Nurse, a Nurse Practitioner, or a Physician’s Assistant.
To be considered for an unlicensed PROS Specialist position you must possess a bachelor’s degree in human services or a related discipline and at least 2 years of human services experience. Candidates that don’t meet the minimum educational requirements but that are able to demonstrate significant and relevant experience for this position are also encouraged to apply.
Responsibilities:
- Provide evidenced-based interventions and treatment in a group or individual setting to a caseload of 20-30 PROS, HCBS and/or CORE clients.
- Attend ongoing training related to PROS, HCBS, cultural competency, risk assessment, and evidenced based practice.
- Complete timely documentation as required by NYS OMH regulations, including but not limited to: initial screening note, comprehensive assessment, service specific assessment, IRPs, ISPs, monthly notes, and progress notes.
- Participate in outreach, engagement, and discharge planning, case coordination and management and, as needed, crisis intervention.
- Assist clients in increasing their overall functioning through added support and coping skills; assist clients in identifying their strengths and barriers.
- Works collaboratively with staff and other community agencies on behalf of clients and/or the agency.
- Participates, engages, listens, and learns in individual and team meetings and supervisions.
- Responsible to review and adhere to all agency policies and procedures.
- Supports a culture that prioritizes professionalism, kindness, teamwork, staff development, and compassion to ourselves, each other, and our clients.
- Responsible to represent the agency in the community with courtesy and professionalism.
- Other assignments and tasks may be added at the discretion of their supervisor .
