Job Summary:
We are seeking a Licensed Master Social Worker (LMSW) to join our healthcare team. The LMSW will provide social work services to patients by conducting psychosocial assessments, identifying patient needs, and connecting individuals with appropriate support and referral services. The role will involve collaborating with healthcare professionals and helping patients access necessary resources and durable medical equipment (DME) services.
Responsibilities:
- Conduct comprehensive psychosocial assessments of patients.
- Identify social, emotional, and support needs based on assessment findings.
- Provide appropriate referrals to community and healthcare services.
- Assist patients with accessing resources and support services.
- Facilitate referrals for durable medical equipment (DME) when appropriate.
- Collaborate with healthcare professionals to coordinate patient care.
- Support patients and families in navigating available services and resources.
- Maintain accurate and timely documentation of social work assessments and interventions.
- Provide appropriate follow-up based on patient needs.
Skills & Experience:
- Minimum 1 year of LMSW experience required.
- Experience conducting psychosocial assessments.
- Experience providing referral services.
- Strong communication and interpersonal skills.
- Ability to work effectively within a multidisciplinary healthcare environment.
Education:
- Master's Degree in Social Work (MSW) required.
License & Certification:
- Valid Licensed Master of Social Work (LMSW) license issued by the New York State Education Department (NYSED).
- Primary Source Verification required.
