Salary: $53,000 - $54,000 annually (Commensurate with experience)

Since 1993, SEPA Mujer has been a vital grassroots non-profit organization dedicated to social change, women’s rights, immigrant rights, and other crucial issues. As the only Latina Rights Organization of its kind, we provide safe spaces for empowerment, leadership development, civic engagement, and legal assistance. Our impactful work is achieved through unique training, workshops, monthly meetings, presentations, tabling, press work, and action-oriented engagement.

Position Summary:
The Community Health Liaison / Enlace de Salud Comunitaria will develop and facilitate program activities and partner with local healthcare providers for SEPA Mujer’s Vida Digna: Respuesta Comunitaria project. Responsibilities include tracking participant progress, providing individualized support, and ensuring successful skill development. The individual will attend outreach events, promote the program together with the Program Coordinator, conduct presentations, and assist with workshops and classes. Additionally, they will manage social media posts and reach out to organizations and agencies that could benefit from the program, working in collaboration with and under the direction of the Program Coordinator and Program Officer.
Key Responsibilities & Essential Functions:
Program Facilitation, Education & Navigation
- Develop and deliver bilingual workshops for community members on preventive health, mental health, nutrition, financial empowerment, computer literacy, and employment readiness.
- Provide individualized resource navigation and case support to guide participants through healthcare, mental health, housing, public benefits, and employment resources.
- Track individual participant progress, monitor skill development, and provide follow-up care coordination to ensure warm handoffs and service completion.
- Operate and respond to calls through the Ayuda Latina hotline, conducting intake, needs assessments, and direct referrals.
- Coordinate transportation assistance (rideshare, bus tickets) and emergency housing/Safe Night Hotel placements for participants, particularly survivors of gender-based violence and isolated farmworkers.
Community Outreach, Partnerships & Engagement
- Conduct direct outreach in rural and geographically isolated areas of Suffolk County (e.g., East End, Hampton Bays, Riverside), including a minimum of 20 farm outreach visits and a minimum of 15 community events.
- Partner with local healthcare providers (such as Northwell Health and Sun River Health) to facilitate access to screenings, preventive care, and health education.
- Deliver a minimum of 10 "Know Your Rights" sessions in partnership with the Eastern Farm Workers Association to inform farmworkers of legal protections.
- Manage social media content and digital campaigns to engage approximately 10,000 individuals with culturally relevant health information and program resources.
- Support the recruitment and facilitation of the Community Advisory Group to gather participant feedback and guide service delivery.
Data Tracking, Compliance & Administration
- Maintain accurate case notes, hotline logs, workshop attendance records, and transportation/voucher tracking reports in database systems (e.g., EmpowerDB).
- Assist in evaluation activities by administering pre-/post-workshop surveys, collecting participant feedback, and generating data reports for project reporting.
- Participate in staff meetings, training sessions, and organizational events under the supervision of the Program Officer.
Qualifications & Requirements:
- Bilingual Fluency: Fully bilingual in English and Spanish (written and spoken) required. Fluency in Indigenous languages is a plus.
- Experience: 2+ years of experience in community health, case management, health navigation, social work, or community organizing, ideally working with immigrant populations, farmworkers, or survivors of trauma.
- Community Connection: Lived experience or deep cultural responsiveness working with Latino/a/x communities, agricultural workers, and rural populations in Long Island.
- Core Skills: Excellent communication, public speaking, workshop facilitation, and relationship-building skills. Proficiency in social media management and basic digital tools.
- Transportation: Valid Driver’s License and reliable personal vehicle for local outreach travel throughout Suffolk County (mileage reimbursed).
- Flexibility: Ability to work flexible hours, including occasional evenings or weekends for community outreach events and workshops.
