In this role, you’ll serve as both a clinical advocate and care navigator — helping members access the right care at the right time while collaborating with physicians, care teams, specialists, and health plan partners to deliver exceptional outcomes and experiences.

What You’ll Do
As a Collaborative Care Nurse, you will:
- Conduct member assessments and preliminary clinical reviews through phone, text, email, and virtual appointments
- Evaluate member needs based on symptoms, medical history, complexity, and prior care
- Coordinate care plans and follow-up activities to ensure timely access to providers and services
- Help members understand and navigate healthcare benefits and treatment pathways
- Advocate for members with health plan partners and care providers
- Collaborate with Direct Primary Care providers, physicians, specialists, pharmacists, and care teams to coordinate treatment plans
- Identify barriers to successful care, including social determinants of health, affordability concerns, and access challenges
- Partner with the Clinical Risk Lead on escalations and complexity management
- Work closely with Care Coordinators to manage and resolve member cases with exceptional service and documentation standards
- Support continuous improvement initiatives that enhance the member experience and healthcare outcomes
- Maintain accurate and compliant clinical documentation
What We’re Looking For
- High school diploma or equivalent
- Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
- Ambulatory care or clinic-based nursing experience
- Strong clinical judgment and ability to multitask effectively
- Excellent communication and interpersonal skills
- High emotional intelligence and the ability to navigate sensitive member interactions
- Strong organizational and problem-solving abilities
- Experience using clinical documentation systems and office productivity tools
Preferred Qualifications
- Experience in medical insurance, chronic case management, or care navigation
- Familiarity with employer-sponsored health plans and benefit structures

Pay: $30.00 - $40.00 per hour
