Conduct assessments, treatment planning, evaluation of the response to care and readjustment of the treatment plan as needed

Plan and provide community education to patients, families and peers

Completes accurate and timely documentation of patient care treatments in EHR (i.e., HomecareHomeBase)

Determine a PT treatment plan

Corona and surrounding cities.

CA Physical Therapy License
-
2 years of physical therapy work experience
-
American Heart Association Basic Life Support (BLS) certification
Job Type: Part-time, Per Visit
Pay: $56.00 - $140.00 per visit
