JOB DUTIES
- Conduct initial and periodic assessments of home health pediatric patients to evaluate speech, language, cognitive-communication, and swallowing functions.
- Re-evaluate the patient’s need for speech therapy services and assess outcomes of care on a regular basis, adjusting the plan of care as necessary.
- Develop and revise individualized plans of care based on evaluation results to ensure quality and continuity of care.
- Identify speech, language, voice, fluency, feeding, and swallowing deficits and implement appropriate therapeutic interventions.
- Provide caregiver education and home programs to support carryover and progress.
- Evaluate the home environment and provide recommendations to enhance communication and feeding safety.
- Assess the need for adaptive equipment, AAC devices, or other assistive technology and provide recommendations as appropriate.
- Follow infection control procedures in accordance with OSHA guidelines to ensure the safety of both staff and patients.
- Coordinate services and communicate with physicians and interdisciplinary team members regarding changes in patient condition or needs.
- Supervise, guide, and provide clinical direction to speech therapy assistants (SLPAs) as required, ensuring quality care and adherence to professional standards.
- Participate in professional development through in-services and continuing education to maintain clinical competency.
- Provide effective communication to patients/clients, family members, team members, and other healthcare professionals.
QUALIFICATIONS
- Graduate of an accredited Speech Language Pathology program with Master’s or Doctorate degree
- Certificate of Clinical Competence (CCC-SLP) preferred
- One year of experience as a Speech Language Pathologist in a pediatric clinical care setting preferred
- Pediatric home health experience preferred
- Reliable transportation
- Current Texas State license as a Speech Language Pathologist
- Current Driver’s License
