Full Job Description

LaSante Health Center is seeking a dedicated HIV Community Liaison to support individuals living with HIV and enhance community engagement. The ideal candidate will have strong communication skills, knowledge of HIV-related issues, and a passion for advocacy.

Responsibilities:

Support individuals living with HIV and connect them to care.
Conduct outreach and educational initiatives.
Build relationships with community organizations.
Advocate for patients and address barriers to care.
Provide HIV-related education and resources.
Collaborate with healthcare teams to support patients.
Maintain records and complete assigned tasks.

Qualifications:

Experience in community outreach or healthcare.
Knowledge of HIV prevention and support services.
Strong interpersonal and organizational skills.
Bilingual in Haitian Creole (preferred).
