Hospice LVN
The Hospice LVN is a member of the clinical team and is responsible for delivering professional nursing care to our patients in accordance with the established plan of care. The Hospice LVN offers quality of care when completing face to face time with patients and when completing all documentation. The Hospice LVN complies with all agency policies and procedures.
Responsibilities for Hospice LVN
Use clinical tools, data and other information to complete patient evaluation and hospice care.
Follow and provide updates related to the patient individualized treatment plans.
Follow case and quality management processes in compliance with regulatory guidelines and company procedures.
Provide skilled nursing care, education, and support to maximize the comfort and health of patients and their families.
Qualifications for Hospice LVN
Active nursing license in the state of California
Strong written and verbal communication skills
Demonstrated ability to assess and respond to patient and family needs in a timely manner.
General knowledge of hospice practices.
Basic computer skills, including working knowledge of the Microsoft Office and EMR
