Rite Med Group is collaborating is seeking a full time, experienced, Nurse Practitioner or Physician Assistant for a post-acute rehabilitation, long-term care, and memory care center near Egg Harbor Township, NJ

The pay range for the Nurse Practitioner position is: $150,000.00 - $180,000.00 per year.
Responsibilities:
- Provide direct patient care, assessments, and treatment for residents and short-term rehab patients
- Collaborate with attending physicians, nursing staff, and therapy teams on care planning
- Manage medically complex patients within our specialty clinical programs
- Support hospital readmission prevention and smooth care transitions
- Document thoroughly and communicate with families as part of a resident-centered approach
- Discussing and reviewing patients' medical history, symptoms, allergies, and current medications
Requirements:
- EMR experience
- Licensed NP/PA in NJ
- At least 1 year experience in an urgent care setting or skilled nursing home experience (Required)
