Job Overview
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) specializing in Hospice care to join our healthcare team. This role involves providing high-quality, patient-centered care to individuals in their final stages of life, ensuring comfort, dignity, and respect. The ideal candidate will have a strong background in various healthcare settings, including long-term care, acute care, and specialized hospice environments, with proficiency in medical documentation and patient care techniques. This position offers an opportunity to make a meaningful difference in patients' lives while working within a supportive and professional team.
Responsibilities
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts.
- Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature.
- Administer medications and treatments as prescribed under the supervision of nursing staff.
- Provide emotional support and companionship to patients and their families during hospice care.
- Maintain detailed medical documentation using EMR/EHR systems while adhering to HIPAA regulations.
- Support patients with specialized needs such as Alzheimer’s/dementia care, memory care, or behavioral health issues.
- Assist with catheter care, wound care, and other clinical procedures following aseptic techniques.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Educate family members on patient needs and provide guidance on home safety and medication management.
- Ensure infection control protocols are strictly followed to maintain a safe environment for patients and staff.
Qualifications
- Valid CNA certification with current licensure in good standing.
- Experience or familiarity with hospice care, palliative care, or end-of-life services preferred.
- Proficiency with EMR/EHR systems such as Cerner or eClinicalWorks is advantageous.
- Knowledge of medical terminology, anatomy, physiology, and basic life support procedures.
- Experience working in diverse healthcare settings including hospitals, nursing homes, assisted living facilities, or home health environments.
- Ability to work compassionately with patients experiencing complex conditions such as dementia/dementia-related illnesses or behavioral health issues.
- Strong communication skills and ability to work effectively within a multidisciplinary team.
- Physical ability to assist with patient mobility and perform clinical tasks requiring lifting or repositioning equipment.
- Commitment to maintaining patient confidentiality in accordance with HIPAA regulations. This position is ideal for caregivers committed to delivering empathetic hospice services in a professional setting that values integrity, teamwork, and excellence in patient care.
