Reports To: Director of Social Work

Responsibilities:

Work with clinical social worker to provide social work services at the service units

Complete psychosocial assessment for patients as indicated
-
Formulate and implement the intervention plan for patients
-
Provide concrete case management, follow-up and referral service
-
Provide entitlements/benefits eligibility counseling and assist with application
-
Assist Clinical Social Workers in providing case management service as needed
-
Timely services provided to patients based on the Social Work department guideline
-
Complete documentations and reports based on the Social Work department guideline
-
Attend case conference, agency meetings and staff meetings
-
Participate in community outreach and marketing activities
-
Perform other duties as assigned
Minimum Qualifications:
-
Bachelor's Degree in Social Science, Social Work, Psychology, Sociology or other related field

At least one year of working experience in social services, health care agency or related field

Good verbal and written communication skills
-
Bilingual in English/Chinese (Mandarin/Cantonese) required
