1. Provides respite, chore, or personal care services to families who are caring for someone who is frail, has Alzheimer’s or related dementia assigned patients, following instructions in the plan of care.
2. Provides services in a respectful and caring manner consistent with patients’ values and lifestyle.
3. Works in a planned, organized and safe manner.
4. Prepare and maintain records and nursing notes of patient progress, services performed, and report changes in patient condition to the Program Coordinator.
5. Weekly phone calls with Program Coordinator to report concerns or problems with patients’ health, and receive guidance and support from the Program Coordinator.
6. Provide assistance and supervision with instrumental activities of daily living (eating, bathing, dressing, toileting, light housekeeping, meal preparation, transferring in/out of bed/chair, walking, and shopping).
7. Keeps patients’ area and personal belongings clean, safe, and sanitary.
8. Complete monthly continuing education units.
9. Submit accurate timecards, signed by the Tribal Council, and accurate time sheets signed by the Client, and Nursing Notes, every two weeks.
10. Follows TCC and Home Care policies and procedures.
