Job Duties and Preferred Qualifications:
A $4,000 hiring bonus is being offered for this position. Hiring bonuses shall only be for new hires of original employment or re-employment as defined in Human Resources Policy No. 5.0.

$1,250 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.

$1,750 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.
The City of Virginia Beach Department of Human Services is seeking to hire a candidate who has experience providing case management services to adults with serious mental health and substance use disorders for the Case Manager position with the Permanent Supportive Housing program in the Behavioral Health Division.
Duties:
Provide clients with enhanced care coordination with equal focus on both behavioral and primary health in a mental health or substance abuse setting.
Complete complex assessments, treatment planning, and crisis planning.
Provide clinical intervention, consultation, supportive counseling, transition planning and other case management duties in a mental health or substance abuse setting.
Provide hospitalized clients with support with discharge planning; link with hospital staff and natural supports as appropriate and obtain medical records to ensure timely follow-up and quality of coordination.
Complete clinical documentation and assure that all filing of clinical information and submission of billing information are done accurately and within the time frame required.
Complete and final approve progress notes in Electronic Health Record (EHR) in timely manner.
Provide leadership and training to team members.
Preferences:
Experience providing direct services to adults with serious mental health and/or substance use disorders.
Experience utilizing community resources in Hampton Roads-Region V serving adults with serious mental illness, substance abuse disorders.
Experience in providing Medicaid reimbursable services to adults with serious mental health and/or substance use disorders.
Experience completing assessments, referrals, & treatment plans for adults with serious mental health and/or substance use disorders.
Experience in risk assessment and determining appropriate clinical level of care for adults with mental health and /or substance use disorders.
Experience utilizing Electronic Health Records to document service delivery.

Minimum Requirements:
MINIMUMS: Requires a four (4) year degree in a human services field (such as psychology, behavioral or mental health, special education, social work) and three (3) years experience in fields providing the required knowledge, skills, and abilities and associated with such positions as Mental Health Counselor, Special Education Teacher, or Social Worker OR any combination of education (above the high school level) and/or experience and training equivalent to seven (7) years in fields providing the required knowledge, skills, and abilities. Must have, or obtain, a current and valid drivers license.
SPECIAL REQUIREMENTS: Must be registered with the Virginia Board of counseling as a Qualified Mental Health Professional (QMHP). All employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies.
