Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patients’ health status by taking vital signs such as blood pressure, pulse, respiration, and temperature
- Administer medications accurately following prescribed instructions while maintaining compliance with home healthcare regulations and HIPAA confidentiality standards
- Support patients with medical procedures such as catheter care, wound care, and other basic nursing tasks under supervision
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient information accurately and securely
- Assist with meal preparation and light housekeeping tasks like cleaning and laundry to ensure a safe and comfortable environment
- Employ proper patient transfer methods using equipment like Hoyer lifts or patient transfer techniques to ensure safety for both client and caregiver
Qualifications
- Proven experience working as a Home Health Aide or in assisted living, nursing homes, or hospice settings
- Strong knowledge of medical terminology, medication administration, and vital signs monitoring
- Experience working with individuals with developmental disabilities, dementia (including Alzheimer's care), or geriatric patients
- Familiarity with home health care assistance protocols, including patient transfer methods and end-of-life care practices
- Ability to operate EMR/EHR systems efficiently for medical documentation purposes
- Certifications in First Aid, Basic Life Support (BLS), CPR are required; additional certifications such as hospice care or dementia training are a plus
- Excellent communication skills for effective client interaction and team collaboration
- Physical ability to perform heavy lifting when necessary and assist with patient mobility safely
