Position Overview:
The Team Leader will provide support and oversight to the Day Program. This may include but not limited to: providing quality supports and programming, assist in the development of service plans for social and interpersonal skills based on assessed needs; responsible for developing a comprehensive service plan, and monitor participants progress against habilitation plan objectives. This person will be responsible for the overall operation of the facility, including: work and training programs, schedules, required reports, supplies, equipment, safety, quality control, acts as a liaison with other agencies, working with our participants, and other functions that may be required to provide appropriate programs. This position plays a critical role in Day Services by supporting sasi’s mission, vision, and core values.

4. Through observation, evaluates participant’s functioning and potential for social, interpersonal, and adaptive skills.
5. Responsible for development of general habilitation plan for social and adaptive remediation.
6. Based on assessed needs, develops specific goals and behavioral objectives for staff action plans.
7. Conducts evaluations/assessments for individuals entering the facility.
8. Develops staff schedules and duty expectations.
9. Responsible for facility-based programs as assigned.
10. Regularly monitors individuals progress against individual service objectives for revision of service plan, recommendation for termination of service, transfer, or referral to other programs.
11. Assists in establishing and maintaining relationships with community resources, referral agencies, and supportive service in order to facilitate referral, linkage, and individual service plan implementation.
12. Maintains required services, statistical and administrative records.
13. Conducts facility-based staff meetings to ensure consistent communications to facility program staff.
14. Participates in administrative and committee meetings, incident review, and other assigned committees.
15. Provides supervision and evaluation to Day Service Manager and Direct Support Professionals.
16. Coordinates facility maintenance with Director of Production and Service Operations.
17. Monitors and maintains training of assigned staff.
18. Responsible for overall administrative supervision at site.

Required Qualifications:
- Bachelor’s degree in health or human series field, PLUS 2 years full-time paid experience in habilitation setting or related service AND 1 year of successful supervisory experience.

7. Able to perform basic math functions (i.e., addition, subtraction, multiplication, division, and percentages).
8. Ability to define areas for improvement, collect data, establish facts, draw valid conclusions, solve practical problems, and deal with a variety of concrete variables in situations where only limited standardization exists.
9. Ability to read data and draw conclusions from it to determine approach or treatment.
10. Ability to use google drive and Microsoft Outlook

Working conditions:
- Must be able to maintain regular work schedule of 35 hours per week as well as perform essential duties as described on attached form.
Travel required:
- Travel between sites, to meetings, etc., during the workday may be required.
- Valid NYS Driver’s License is required.
Training expectations:
- ¬¬You are required to maintain your certifications and annual trainings, as applicable to your role.
