Serving the community since 1989, JPHSA helps individuals and families in Jefferson Parish affected by mental illness, addictive disorders, and developmental disabilities to live full, healthy, independent, and productive lives. JPHSA is a Louisiana Local Governing Entity (LGE) and special district established by state law to administer behavioral health, developmental disabilities, and related services in Jefferson Parish. JPHSA is governed by an independent Board of Directors and is not a department of Jefferson Parish government.

Responsibilities
- Supervises care coordinators within a chain of command, including providing coaching, mentoring, and performance evaluations
- Completes audits of care coordinator caseloads and assignments
- Monitors and ensures tracking of external and internal referrals
- Assists with complex cases and ensures the quality of referral management, follow-up, and communication among all parties
- Develops monitoring reports for presentation to senior staff
- Implements distribution and reporting for satisfaction surveys
- Participates in external and internal stakeholder staff outreach meetings
- Helps remove barriers to care (transportation, housing, insurance, etc.)
- Coordinates with providers, hospitals, community agencies, and family members
- Ensures timely completion of assessments and care plans
- Tracks outcomes and performance measures
Qualifications and Skills
- A bachelor's degree and three years of social service experience, or an advanced degree in a social science field and two years of experience, or six years of social service experience
- Clinical knowledge preferred, including the ability to identify high-risk, high-need clients, support care coordination efforts across programs, and provide guidance to non-licensed staff (professional licensure not required)
- Proficient in the use of electronic health record (EHR) systems
- High ethical standards and personal integrity
- Strong organizational and time-management skills with exceptional attention to detail
- A self-starting problem-solver who can think critically and improve processes
- Professional, polished communication skills
- Ability to build strong working relationships with individuals served, colleagues across departments, and local providers and stakeholders
- Commitment to interacting in a culturally competent way and consistently treating all with dignity and respect
