Responsibilities
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient health status, documenting vital signs and changes accurately in electronic health record (EHR) systems
- Support medication management by administering prescribed medications following medical instructions and home healthcare regulations
- Provide specialized care such as catheter care, dementia and Alzheimer's care, and end-of-life support with sensitivity and professionalism
- Help with meal preparation, light housekeeping, laundry, and maintaining a clean and safe living environment
- Use patient transfer methods such as Hoyer lifts or other assistive devices to ensure safe mobility and prevent injury
- Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and ensure patient confidentiality in compliance with HIPAA standards
Requirements
- Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care settings
- Knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (CPR/First Aid) certifications
- Familiarity with EMR (Electronic Medical Records) systems and medical documentation practices
- Ability to lift heavy objects safely and perform physical tasks required for patient transfer and mobility assistance
- Experience working with individuals with developmental disabilities, dementia care, geriatrics, or hospice care preferred
- Strong communication skills to provide compassionate patient care and client interaction
- Valid certification in CPR/First Aid; additional certifications such as Hoyer lift training or hospice care experience are advantageous
- Adherence to home health care regulations and maintaining patient confidentiality at all times
