Job Title: Licensed Practical Nurse (LPN/LVN) – Home Health/Hospice
Location: Asheville, NC
Shift: 8 Hours Mon – Fri 9 – 5
Guaranteed Hours: 40 Hours/Week
Contract Duration: 90 Weeks
Position Summary
The Licensed Practical Nurse (LPN/LVN) administers skilled nursing care to patients in their place of residence as prescribed by the physician and in accordance with the established plan of care. The LPN/LVN works under the supervision of a Registered Nurse (RN) and coordinates care with the interdisciplinary team as well as the patient and family. Care is delivered in compliance with State, Federal, and Medicare standards.
Responsibilities
- Assist the RN in providing skilled nursing care and assessing patients' physical, psychosocial, and environmental needs.
- Support the development and implementation of individualized patient care plans.
- Communicate effectively with patients, families, and the healthcare team, documenting all care and interventions.
- Educate patients and caregivers on self-care techniques and treatment plans.
- Follow OSHA infection control and safety protocols.
- Assist with hospice and end-of-life care under the direction of the RN and physician.
- Support patient admissions, discharges, and transfers.
- Participate in on-call rotations as assigned.
- Maintain accurate electronic documentation using Homecare Homebase (HCHB).
- Maintain professional standards, patient confidentiality, and workplace safety.
- Participate in required training, performance improvement activities, and other assigned duties.
Required Qualifications
- Graduate of an accredited Licensed Practical Nursing (LPN) Program or accredited School of Vocational Nursing.
- Current, unrestricted LPN/LVN license in the practicing state.
- Current CPR Certification.
- Minimum of two (2) years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
- Experience with Homecare Homebase (HCHB) electronic medical record system.
- Valid driver's license.
- Reliable transportation and current auto liability insurance.
- Current Hepatitis consent/declination and TB vaccination documentation.
- Ability to read, write, and speak fluent English.
