Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) – Home Health Nurse / Registered Nurse (RN)! Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications via injections, IV infusions, catheterizations, and other methods while adhering to HIPAA regulations and sterile techniques.
- Develop and implement individualized care plans for patients with complex needs such as dementia, Alzheimer's disease, or chronic conditions like pain management or infectious diseases.
- Provide specialized care including wound dressing changes, tube feeding management, ventilator support, and post-acute care procedures.
- Conduct patient education on medication administration, disease management, nutrition, and self-care strategies tailored to diverse populations including pediatrics, geriatrics, and individuals with disabilities.
- Collaborate with physicians and other healthcare professionals for diagnostic evaluations, triage assessments, discharge planning, and case management to optimize patient outcomes.
- Maintain accurate medical documentation of patient interactions, treatments administered, and progress notes in compliance with healthcare standards such as NCQA or JCAHO.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of practice.
- Proven experience in various clinical settings such as hospital medicine, ICU or trauma centers (Level I or II), emergency medicine, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with ICD-9/10 coding is preferred.
- Critical care experience including ICU or emergency room work is highly desirable; experience with infectious disease care and acute pain management is a plus.
- Strong knowledge of anatomy, physiology, medical terminology and diagnostic evaluation techniques.
- Ability to perform complex procedures such as phlebotomy, suturing, spinal taps or cardiac catheterization when necessary.
- Excellent patient assessment skills with a focus on safety protocols like basic life support (BLS), airway management and infection control practices.
- Experience working with diverse populations including children (working with children/toddlers), seniors (geriatrics), individuals with developmental disabilities or memory impairments such as dementia or Alzheimer’s care.

Pay: $38.71 - $46.62 per hour
