Person Directed Supports Inc. is a Human Services Agency who supports Individual's with intellectual and developmental disabilities. We are looking for a Medical Service Coordinator to assist with the continued coordination of medical appointments and needs of the Individuals. JOB RESPONSIBILITES:
- Creates/promotes a culture centered on building self-directed lives with the people we support.
- Demonstrates effective administration of assigned units; complies with established administrative policies and duties.
- Displays principles of good management and applies these principles in daily work performance.
- Coordinates allotted work schedule and assigned duties
- Schedules medical appointments in accordance to licensing guidelines, individual needs, and as per doctor recommendations.
- Keeps track of all doctor visits on monthly doctor visit forms and gives to Program Specialist at the end of the month for monthly recording purposes.
- Makes certain medical casenotes are completed and accurately filed
- Ensures medical appointment recommendations and follow-up are completed
- Works closely with agency medical personnel to ensure compliance and follow through on any individual medical concerns or physician recommendations.
- Reports to and assists Medical Director.
- Notifies the Medical Director and Director of Risk Management or agency Nurse in an accurate and timely manner of unusual incidents, problems, and changes that require immediate attention.
- Review MARs and maintain close communication with pharmacies
- Working closely with insurance companies to ensure medical needs are covered.
- Do on site house checks to ensure that all medications have been given as ordered, order refills on all the medical supplies, and that physician’s recommendations are being adhered to and documented.
- Communicate with family and county to ensure the best possible care for each individual. Update the support team, in conjunction with your supervisor, with any new medical conditions, concerns, or incidents.
- Abides by established policies and procedures of the agency, County and State; is responsible for direction and supervision for compliance of medical issues.
- Promotes a positive working relationship with all employees.
- Assists in the planning and development of the Person Centered Plan.
- Provides direct support as needed.
- Assist in keeping track of all individuals’ physical, dental, vision, and all immunizations, vaccines, and screenings.
- Create forms necessary to complete job duties such as medication administration tracking records, turn sheets, seizure logs, etc.
- Other duties as assigned by agency
Job Requirements:
- Medical background with a Bachelor’s Degree or Associates Degree from an accredited College or University.
- Experience working with medical insurance companies.
- Minimum of two years’ experience working directly with individuals with Intellectual/Developmental Disabilities.
- Driver’s license required with the ability to maintain a good driving record.'
