Job Summary
We are seeking a compassionate, organized, and experienced Resident Care Coordinator to oversee operations across eight Family Care Homes. This leadership role is ideal for someone with experience in assisted living or family care homes who is passionate about providing quality resident care while supporting occupancy growth.
Key Responsibilities
* Coordinate resident care services across eight Family Care Homes.
* Conduct resident assessments and ensure individualized care plans are implemented.
* Coordinate admissions, discharges, and resident transitions.
* Build and maintain relationships with hospitals, rehabilitation centers, case managers, and referral sources to increase admissions.
* Market the homes to healthcare professionals and community partners to maintain high occupancy.
* Ensure compliance with state regulations and company policies.
* Provide guidance and support to caregivers and home managers.
* Monitor resident satisfaction and address concerns promptly.
* Maintain accurate resident records and documentation. Qualifications
* Experience in Assisted Living, Family Care Homes, or Long-Term Care preferred.
* Knowledge of admissions, discharge planning, and resident assessments.
* Excellent communication, organizational, and leadership skills.
* Marketing or healthcare referral experience is a plus.
* Valid driver’s license and reliable transportation (travel between homes required).
* Knowledge of North Carolina Family Care Home regulations is preferred.
