Job Summary and Responsibilities
As our Senior Patient Logistics Coordinator, you will be a critical leader in patient care coordination within our Virtual Hospital in Phoenix, AZ. You will apply critical thinking and problem-solving to deliver effective care to patients, providers, and leaders, collaborating closely with the Clinical Command Center (CCC) team in a safe, confidential environment.
Every day you will utilize strong communication and your understanding of the care continuum to identify patient and team needs. You will efficiently guide resources or execute workflows to progress patients timely through care using designated technology. You'll also support CCC leaders in Quality Assurance, providing feedback for individual and departmental improvements to meet established goals.
To be successful in this role, you will: demonstrate excellent communication skills, a deep grasp of care continuum needs, and patient experience outcomes. Your ability to identify needs, guide resources, and drive patient progression, while also contributing to QA processes and supporting CCC growth initiatives, is essential for care innovation and professional development.

Functions as a lead resource to the CCC team; subject matter expert on the multiple applications and technology used to support patient care through the care continuum.

Provides care for patients under services in the clinical command center.

Monitors and appropriately escalates any delays in patient care transition management.

Monitors RightFax folder to ensure all faxes are managed timely and appropriately.

Ensures Command Center Displays are functional and updated, at least weekly.

Ensures that reports are completed timely and accurately.
Job Requirements
Required
-
Associates Other and or equivalent work experience demonstrating leadership function
-
MA, CNA, LVN or or equivalent experience in a health care setting and demonstrated clinical competencies related to clinical command center functions
Preferred

2+ years working in a healthcare setting, preferably in bed placement and/or transfer center capacity

Pay Range
$25.55 - $38.00 /hour
