Job Summary
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our home care team. In this vital role, you will provide personalized nursing care directly in patients’ homes, ensuring comfort, safety, and optimal health outcomes. Your expertise will support individuals with diverse needs, including those requiring chronic disease management, post-acute recovery, or specialized dementia and memory care. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications and injections accurately following prescribed protocols while adhering to HIPAA regulations and medical documentation standards.
- Manage complex wound care, including dressing changes and sterile procedures.
- Provide specialized care for individuals with Alzheimer’s disease, dementia, or developmental disabilities, ensuring safety and dignity.
- Support patients tube feeding in a home setting.
- Collaborate with physicians and case managers to develop and implement personalized care plans aligned with patient goals.
- Educate patients and their families on medication management, health coaching strategies, and disease processes to promote independence.
- Respond promptly to urgent situations through triage protocols, including acute pain management or infectious disease concerns.
- Ensure infection control practices are maintained at all times to safeguard patient health in compliance with healthcare standards.
Requirements
- Valid LVN license issued by the state licensing board; current CPR/BLS certification required.
- Proven experience in home care settings.
- Strong knowledge of physiology, anatomy, pharmacology related to medication administration, and diagnostic evaluation techniques.
- Experience working with individuals with disabilities or behavioral health needs; familiarity with Alzheimer’s/dementia care is required.
- Experience working with patients with type 1 diabetes is required.
- Excellent communication skills for effective patient assessment and collaboration within multidisciplinary teams.
- Flexibility to work independently in a home environment while maintaining high standards of patient safety and confidentiality under HIPAA guidelines.
Job Type: Contract
Pay: $27.25 - $35.00 per hour
Expected hours: 40 per week
