Position Summary

Lighthouse Senior Care Consultants is seeking a compassionate RN Geriatric Care Manager to help aging adults and their families navigate complex healthcare, home care, facility transitions, long-term care needs, and quality-of-life decisions.

Essential Job Functions
The RN Geriatric Care Manager provides professional care management services for adult and aging clients who need unbiased, knowledgeable, and compassionate support. Responsibilities include, but are not limited to:
· Conduct comprehensive assessments of older adults and adults living with chronic illness, cognitive changes, disability, or complex psychosocial needs.
· Develop, implement, and update individualized care plans that address client goals, safety, health needs, support systems, and quality of life.
· Coordinate medical appointments, home care services, community resources, facility transitions, discharge plans, and ongoing care needs.
· Monitor changes in health status, medication adherence, functional abilities, care plan effectiveness, and family concerns.
· Serve as a liaison between clients, families, physicians, healthcare providers, home care agencies, facilities, attorneys, financial professionals, and other members of the care team.
· Provide education and guidance to clients and families about chronic conditions, medications, dementia care, safety concerns, care options, and available support resources.
· Advocate for clients in healthcare, residential, and community settings to promote dignity, safety, independence, and appropriate care.
· Conduct routine home, facility, hospital, and community visits based on client needs.
· Transport clients to medical appointments or appropriate community outings when needed; a reliable personal vehicle is required.
· Document assessments, care plans, client updates, provider communication, and follow-up needs accurately and timely.
· Attend bimonthly in-person staff meetings and company trainings as required.
· Participate in a rotating after-hours on-call schedule, approximately every 6–8 weeks, to support urgent client needs.
Education, Experience, and Requirements
· Active Registered Nurse (RN) license in Virginia, in good standing, required.
· Minimum of 3 years of clinical nursing experience required.
· Experience in geriatrics, home health, hospice, case management, discharge planning, chronic disease management, dementia care, care coordination, or Aging Life Care preferred.
· Experience supporting older adults, adults with chronic illness, individuals living with dementia, and families or caregivers preferred.
· Knowledge of Medicare, Medicaid, long-term care systems, private insurance, entitlement programs, and community resources preferred.
· Strong assessment, critical thinking, organization, problem-solving, and communication skills.
· Ability to work independently, manage multiple clients, prioritize competing needs, and navigate complex care situations.
· Reliable vehicle, valid driver’s license, safe driving record, and willingness to travel locally throughout Northern Virginia required.
· Proficiency with Microsoft Office, Outlook, smartphone use, web-based documentation systems, and electronic records required.
· Ability to pass a background screening required.
· Candidates must currently reside in Fairfax County, Arlington County, Alexandria, Loudoun County, or Prince William County.

Schedule and Work Expectations
· Full-time and part-time opportunities available.
· Work is primarily daytime and community-based, with local travel required during work hours.
