Elder Options is looking for a a compassionate and organized professional to join our diverse team of of nurses, social workers, and gerontologists in a Social Worker/Case Management Support role serving participants in the Assisted Living Waiver (ALW) program.
This position is an opportunity for someone who enjoys working directly with older adults, coordinating services, completing visits and documentation, and helping individuals remain safely and successfully in the community.
About the Role
The Social Worker/ Case Management Support professional will assist our care management team with Monthly Visits (MHVs) and related participant support and documentation.
Responsibilities may include:
- Conducting in-person Monthly Visits with ALW participants
- Developing positive, professional relationships with participants and their support networks
- Assessing participants' needs, identifying changes in condition or circumstances, and communicating concerns to the appropriate members of the team
- Speaking with caregivers, families and other service professionals as needed
- Completing required visit documentation accurately and within established timelines
- Maintaining timely and thorough documentation in electronic systems
- Following ALW program requirements, policies, and procedures
- Communicating effectively with the team
- Traveling throughout the Sacramento County to meet with participants in assisted living facilities
- Other case management and participant-support duties as assigned
Qualifications
We are open to candidates with a bachelor's degree or higher in a related field, including:
- Social Work
- Sociology
- Psychology
- Rehabilitation
- Human Services
- Gerontology
- Or another closely related field
Required:
- At least 1 year of case management experience
- Experience working with older adults, individuals with disabilities, or other vulnerable populations preferred
- Strong communication and interpersonal skills
- Excellent organization and attention to detail
- Ability to work independently while remaining an effective member of a multidisciplinary team
- Reliable transportation and a valid driver's license
- Ability and willingness to travel throughout Sacramento County for visits

Pay: $30.00/visit + mileage reimbursement
SCHEDULE AND LOCATION
- Part-time position with flexible schedule
- You determine your visit schedule based on client needs and availability
- Client visits are in person throughout Sacramento County
- A monthly staff meeting at the Elder Options office in Placerville
- Confidential documentation is completed electronically following visits on a home computer
