Oversee and coordinate all social services functions for residents
Complete psychosocial assessments, care plans, and progress notes in accordance with state and federal regulations
Assist residents and families with adjustment, discharge planning, and community resource coordination
Participate in care plan meetings and interdisciplinary team conferences
Support residents with advance directives, grievances, and rights advocacy
Coordinate discharge planning and transitions of care
Maintain accurate, timely, and survey-ready documentation
Collaborate with nursing, therapy, administration, and external agencies
Ensure compliance with CMS, state regulations, and facility policies
