Behavioral Health Agency seeking an Intensive In-Home Team Lead.
Education/Experience Qualifications for Team Lead position (Required):
*Masters Degree in Human Service Field
*Provisional License status or licensed clinician in the state of NC (LPC, LCSW or dually licensed professional including the above, and LCAS license)
*Minimum of one year post-graduate experience working with target population
*License Has no substantiated findings of abuse or neglect listed on the North Carolina Health Care Personnel Registry or Central Registry
Duties/Responsibilities Include (but are not limited to):
*Administer therapeutic interventions (i.e. modeling, behavior modification, behavior rehearsal) for the development of interpersonal skills, community coping skills and independent living skills that aid in the development of therapeutic consumer symptom monitoring and management. Provide consumer psycho-education and support.
*Complete service notes within 24 hours of service.
*Attend and actively participate in weekly IIH team meetings, staff meetings, and training as required to maintain current certification/license in all required training as outlined by agency policy and/or state and federal regulations according to discipline and service.
*Assess clinical needs, monitoring psychiatric and addiction symptoms, medication compliance and Medical/Mental Health treatment compliance; and consulting with identified medical (for example, primary care and psychiatric) and non-medical(for example, DSS, school, DJJDP) providers, to engage community and natural supports, to include their input in the person-centered planning process to ensure linkage to the most clinically appropriate service including arranging psychological & psychiatric evaluations.
*Participate/facilitate Child and Family Team Meetings monthly
*Assist/review and revise Person Centered Plans and the ongoing monthly plan updates.
*Demonstrate knowledge of emergency procedures; develop crisis plans & update as needed. Support consumers in crisis situation; implement crisis plan; and participate in on-call first responder crisis response coverage as required.
*Assist in the coordination of services across levels of care including intake, orientation, therapy, crisis management, case management, client education, scheduling, attending appointments, attending meetings, referrals, etc. for the youth or other members of the family with other licensed professionals.
*Accurate completion, monitoring, tracking and updating of required paperwork including prospective and concurrent authorizations, NC TOPPS, PCPs, Surveys, team schedules, etc. Complete all documentation in a timely manner.
*Effectively facilitate consumer access to community resources to assists consumer with developing natural supports, locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, and providers to teach life skills relevant to mental health, developmental disabilities, and substance abuse services.

Pay: $60,000.00 - $70,000.00 per year
