About Community Pathways
Community Pathways, Inc. (CPI) is a support services brokerage that provides case management services to adults who experience an intellectual and/or developmental disability (I/DD) and live in their own home or family home. We serve over 500 individuals living in Clackamas, Multnomah, and Washington counties in the Portland Metro area of Oregon. Our mission is to assist people with disabilities to live empowered, self-determined lives through community connections.
About Personal Agents (Case Managers)
Personal Agents have a caseload of 38 to 40 adults. The individuals we support live in Multnomah, Clackamas, and Washington counties, and they reflect the diverse fabric of these communities. People experience disability across class, race, gender, and ethnic identities. Some of the people we serve live with their families or friends, others experience homelessness, and others live in their own homes and apartments. We work with people who experience a wide array of disabilities, and some may also experience mental health conditions and addictions.
Personal Agents assist individuals (and their support teams) to:
- Navigate the support services system, using person-centered planning techniques to assess their needs and develop plans and goals;
- Secure resources and services (in areas such as employment, housing, crisis, and direct support providers);
- Address health and safety needs; and
- Monitor the supports they receive.
Personal Agents advocate with and on behalf of the people on their caseload. They provide problem-solving (sometimes on an urgent basis), and manage intensive paperwork; including plan development, progress notes, needs assessments, and correspondence. They also monitor the quality of services, and ensure choice and independence.
Position Type and Working Conditions
This is a 40-hour a week non-exempt position. Typical schedule will be Monday through Friday, 8:30am to 5pm but may vary depending on customers’ needs. Additional hours may occasionally be required.
During your training period, you will be required to work at our office on a regular basis located in inner SE Portland. Once you are fully trained, you may have the opportunity to work a hybrid schedule.
Required Qualifications
- An undergraduate degree in a human services field and at least one year of experience in the area of developmental disabilities; OR
- Five years of equivalent training and work experience related to developmental disabilities; AND
- Knowledge of the public service system for developmental disability services in Oregon.
- Ability to pass a criminal background check per OAR 411-340-0070 (1) (b) (e) (f).
- Access to a working vehicle (or acceptable alternate means of transportation), good driving record, required auto insurance, and the ability to safely and efficiently meet individuals in their homes and other community settings in our service area.
Desired Characteristics
- At least one year of case management or program management experience.
- Excellent written and verbal communication skills.
- Strong interpersonal skills and experience in mediation or conflict resolution.
- Skilled with Microsoft Word, Excel, and Outlook.
- Ability to prioritize, meet deadlines, and manage multiple projects in a fast-paced environment.
- Commitment to customer self – determination and personalized support.

- Pay starts at $59,500 per year with an additional increase after a successful 90-day introductory period.
