What You'll Do
- Evaluate and treat patients in their homes
- Develop individualized treatment plans
- Improve mobility, balance, strength, and independence
- Help prevent falls and hospitalizations
- Educate patients and caregivers
- Collaborate with nurses, occupational therapists, speech therapists, physicians, and social workers
What We're Looking For
- Graduate of an accredited Physical Therapy program
- Active Colorado Physical Therapist license
- Current CPR/BLS certification
- Valid driver's license and reliable transportation
Home health experience is welcome but not required.
