PrimeCare Home Solutions is actively seeking compassionate and dedicated Caregivers to join our team to assist with a Female Client who lives off of 20th Street/Glendale Ave. Client has MS and is non-ambulatory. She is of small stature and requires a lift transfer. The ideal candidates will have experience caring for patients with MS and Scholiosis or similar ailments, and a genuine desire to provide exceptional patient-centered care, supporting individuals with diverse needs, including seniors, those with developmental disabilities, and individuals experiencing memory loss or requiring hospice support. Duties
- Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support.
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records in compliance with HIPAA regulations.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation.
- Support medication administration and ensure proper documentation in accordance with care plans.
- Provide companionship and emotional support to reduce feelings of isolation and promote mental well-being.
- Maintain a clean and safe environment through light housekeeping tasks including cleaning, laundry, and organizing personal spaces.
- Utilize specialized equipment such as Hoyer lifts or other assistive devices to facilitate safe patient transfers and mobility.
- Collaborate with healthcare professionals to develop and implement personalized care plans that meet each resident’s unique needs.
- Engage in patient observation to identify changes in condition or behavior that require escalation or intervention.
- Support residents with Alzheimer's disease, dementia, autism spectrum disorders, or other developmental disabilities through specialized care techniques such as memory care strategies.
Experience
Candidates should possess prior experience working in home health, assisted living facilities, nursing homes, group homes, or hospice environments. Experience with patient monitoring, medication administration, and care planning is essential. Familiarity with working with individuals with disabilities—including developmental disabilities—and providing dementia or Alzheimer's care is highly valued. Candidates must demonstrate proficiency in vital signs measurement, CPR certification, first aid training, and HIPAA compliance. Previous roles involving heavy lifting or the use of assistive devices such as Hoyer lifts are preferred. A background in social work or nursing support that emphasizes compassionate caregiving and long-term care is advantageous.
