Job Overview
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA) to join our dynamic healthcare team. Duties
- Observe and monitor patients’ conditions regularly, documenting changes accurately using PointClickCare (POC) or Electronic Medical Record (EMR) systems.
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
- Administer medications as prescribed, ensuring proper documentation in compliance with home health regulations and HIPAA standards.
- Perform vital signs assessments such as blood pressure, pulse, respiration, and temperature to track patient health status.
- Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management.
- Support patients during end-of-life or hospice care with compassion, dignity, and respect for their wishes.
- Handle catheter care, wound dressing changes, and other medical procedures under supervision or according to training protocols.
- Assist with light housekeeping duties such as cleaning patient areas and laundry to maintain a safe and hygienic environment.
- Prepare nutritious meals tailored to dietary restrictions and assist with meal consumption when necessary.
- Use assistive devices like Hoyer lifts or other transfer equipment safely to prevent injury for patients and staff.
- Ensure compliance with all home & community care guidelines while maintaining patient confidentiality under HIPAA regulations.
Experience
- Prior experience working as a Certified Nursing Assistant or Personal Care Assistant in assisted living facilities, nursing homes, or home healthcare settings is highly preferred.
- Demonstrated experience working with geriatric patients and individuals with developmental disabilities or disabilities requiring specialized support.
- Familiarity with medical terminology, medication administration protocols, and medical documentation practices is essential.
- Experience utilizing EMR/EHR systems for accurate record keeping is advantageous.
- Knowledge of dementia care techniques, Alzheimer’s care strategies, hospice support, and end-of-life procedures is desirable.
- Previous hands-on experience with hospice or palliative care enhances your ability to provide compassionate support during sensitive times.
- Ability to perform heavy lifting safely when required for patient transfers or equipment handling.
- Strong understanding of home health regulations along with excellent caregiving skills rooted in patience, empathy, and professionalism.
