Job Summary

Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates using EMR (Electronic Medical Records) systems
- Support patients with medication administration following medical instructions and safety protocols
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques
- Perform catheter care and assist with hospice or end-of-life care needs with sensitivity and professionalism
- Maintain accurate medical documentation and ensure compliance with home healthcare regulations and HIPAA privacy standards
- Support patients with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
- Use proper lifting techniques and equipment like Hoyer lifts to assist with patient transfers safely
- Observe patient conditions closely for any changes or concerns related to health or safety
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred
- Demonstrated experience caring for geriatric patients or individuals with disabilities in home or community environments
- Familiarity with medical terminology, EHR (Electronic Health Record) systems, and medical documentation practices
- Hands-on experience with CPR and basic life support (BLS), including first aid training
- Experience working with individuals who have developmental disabilities or require specialized dementia or Alzheimer's care
- Knowledge of home & community care standards, hospice care protocols, and end-of-life support practices
- Ability to perform heavy lifting when necessary and operate mobility aids such as Hoyer lifts safely

Pay: $15.00 - $18.00 per hour
