The Role
As our Social Worker, you will lead the social services program for our residents. You will report directly to the Director of Social Services and the Administrator, serving as the primary bridge between clinical care, family support, and community resources.
Key Responsibilities
Resident Advocacy: Identify medically related social needs and provide counseling and support for residents and their families.
Care & Discharge Coordination: Lead quarterly family meetings and coordinate organized discharge plans, connecting residents with vital Stark County community resources.
Clinical Documentation: Maintain high-quality medical records, documenting social data, support networks, and individualized care plans.
Liaison Services: Act as a bridge between residents, families, and outside agencies to ensure resident rights and quality of life are maintained.
Requirements
Education: Bachelor’s degree in Social Work (BSW) or a related human services field (Sociology, Psychology, Special Education, or Rehabilitation Counseling).
Licensure: Must hold a current Ohio Social Work License (LSW) or LISW.
Skills: Excellent verbal/written communication and the ability to work independently in a fast-paced clinical setting.
