The Registered Dietitian (RD) is responsible for assessing residents’ nutritional needs and developing, implementing, and monitoring individualized nutrition care plans in accordance with physician orders, federal and state regulations, and facility policies. The RD plays a key role in supporting resident health, recovery, and quality of life through evidence-based nutrition interventions.
Conduct comprehensive nutrition assessments upon admission and at regular intervals, including significant change and quarterly assessments
Develop, implement, and update individualized nutrition care plans based on residents’ medical conditions, preferences, and goals
Monitor residents for nutritional risk factors such as weight loss, malnutrition, dehydration, and swallowing difficulties
Collaborate with the interdisciplinary care team (physicians, nursing, speech therapy, dietary staff, and social services)
Recommend appropriate diets, supplements, and texture modifications in compliance with physician orders
Participate in care plan meetings, quality assurance/performance improvement (QAPI) activities, and clinical rounds
Provide nutrition education to residents, families, and staff as needed
Review and analyze menu plans for nutritional adequacy and regulatory compliance
Ensure compliance with CMS, state, and local regulations related to nutrition services
Document all assessments, interventions, and outcomes accurately and timely in the medical record
Support survey readiness and respond to deficiencies related to nutrition services, if applicable
