As our agency continues to grow, we are seeking an experienced Client Care Manager to lead client care operations, support our caregivers, collaborate with our scheduling team, and help ensure exceptional service throughout Northern Virginia.
This is a hands-on leadership position where you'll work directly with ownership, have the opportunity to make meaningful decisions, and play an important role in shaping the continued success of our agency.
What You'll Do
Client Care
- Conduct new client intake assessments
- Develop, implement, and update individualized care plans
- Monitor client needs, preferences, and changes in condition
- Maintain accurate client records and documentation
- Advocate for clients and ensure services meet their needs
Client & Family Relations
- Build strong relationships with clients and families
- Serve as the primary contact for client care concerns
- Resolve service issues promptly and professionally
- Promote outstanding customer service and client satisfaction
- Help retain long-term client relationships
Caregiver Leadership
- Support, coach, and mentor caregivers
- Conduct caregiver supervisory visits and performance observations
- Communicate care plan updates and client expectations
- Promote caregiver engagement and retention
Scheduling & Operations
- Work closely with the scheduling team to ensure excellent service delivery
- Assist with complex scheduling situations, caregiver call-offs, and staffing challenges
- Help ensure caregivers are appropriately matched with clients
- Promote continuity of care and consistent caregiver assignments
- Recommend process improvements that enhance efficiency and client satisfaction
Quality Assurance & Compliance
- Monitor service quality and company standards
- Assist with regulatory compliance, audits, and quality assurance activities
- Identify and resolve care-related or operational concerns
- Help ensure clients receive safe, high-quality care
Leadership
- Collaborate closely with ownership and office staff
- Participate in case reviews and operational planning
- Conduct client visits, caregiver supervisory visits, quality assurance visits as needed
What We're Looking For
Required
- Minimum of 2 years of experience in Home Care, Home Health, Hospice, Assisted Living, Skilled Nursing, or Long-Term Care
- Experience in care coordination, client care, scheduling, caregiver supervision, Case Management, or leadership responsibilities
- Strong knowledge of Activities of Daily Living (ADLs), dementia care, home care operations, caregiver supervision, and care planning
- Excellent customer service, communication, leadership, and relationship-building skills
- Strong organizational, problem-solving, and decision-making abilities
- Ability to manage multiple priorities in a fast-paced environment
Preferred
- Experience conducting client assessments and developing care plans
- Experience supervising caregivers, scheduling operations, or care coordination teams
- Bachelor's degree in Healthcare Administration, Social Work, Human Services, Gerontology, Nursing, or a related field
- Previous experience in home care operations
- Experience using home care software such as AxisCare, WellSky, ClearCare, AlayaCare, CareSmartz360, MatrixCare, or similar home care management systems is preferred.
