Care Manager
Community Connection Healthcare, LLC is looking for a Care Manager for the Northshore, Boston, Weymouth/Dedham, Brockton, Taunton, Stoughton/Sharon & surrounding areas. Positions are part time and CCH will work with staff to build a flexible schedule.
This position is 100% on the road!

Essential Job Functions:
- Responsible for on-site visits with each client at their home.
- Refer, request, and/or outsource services as needed.
- Communicate with outside agencies to support AFC/GAFC client requests.
- Completing a care management progress note for each on-site visit or encounter and upon significant change.
- Ensuring that all needed GAFC/AFC information is properly delivered.
- Monday thru Friday phone availability for clients, staff and meetings.
- Ability to work independently and as a team member.
- Detail orientated w/ability to multi-task.
- Skilled in technology/software with strong organizational and planning skills.
Requirements Include:
- A bachelor's degree in a healthcare-related field required
- Bilingual (Spanish/English speaking) preferred
- CPR Certification required
- Updated TB skin test and Physical
- Background check required
- Driver's License/State ID required
- Travel, must have own transportation, flexibility (up to 7 days a week)
- Mature, responsible, attendance, and punctuality
- High standards of honesty, and integrity
- Must be able to work independently with little or no supervision
