Lead Clinical Supervisor for TeleConnected Medicine LLC

About TeleConnected Medicine
TeleConnected Medicine runs chronic care management and advanced primary care management programs for rural hospitals and clinics. We serve 11 organizations across five states with roughly 4,300 patients under management. Our care managers work inside each client's own system, and the client's providers keep the patient relationship. We handle the outreach, the documentation, and the reporting most rural programs cannot staff.
The role
The Lead Clinical Supervisor is responsible for the quality and consistency of the work our care managers do every day. This person makes sure every patient interaction is clinically sound, every record is complete before it reaches a provider chart, and every care manager knows what is expected and is supported in getting there.
This is a hands-on supervision role. Clinical protocols, patient engagement content, and the training curriculum are owned by the Clinical Director. Care plan development and anything requiring independent clinical assessment stay with the Clinical Director as well. This position executes against those standards and reports on what is working.
Core responsibilities
1. Documentation review and clinical quality
- Review care manager documentation and patient interactions for accuracy and completeness before they reach a provider record
- Maintain a defined review sample each month and document what was reviewed
- Escalate clinical concerns to the Clinical Director
2. Care manager supervision
- Provide day to day oversight of care managers across all client programs
- Conduct performance reviews, coaching sessions, and documentation audits
- Identify training gaps and deliver targeted coaching
- Onboard and train new care managers
3. Performance accountability
- Track engagement rate, billable patient rate, care plan completion, and patient satisfaction by care manager and by site
- Hold care managers to monthly performance goals
- Identify underperformance early and implement corrective action plans
4. Weekly clinical leadership
- Lead structured weekly meetings with care managers
- Provide real time clinical guidance, case review, and escalation support
- Serve as the clinical point of contact when a client's provider has a question
5. Reporting
- Report weekly to the Clinical Director on performance, risks, and issues by site
- Contribute the clinical portion of the monthly client and executive reporting package
Location: Nashville or Manchester, Tennessee, with remote flexibility
Travel: Occasional, for client site visits and team meetings
Minimum licensure: Registered Nurse or Licensed Practical Nurse, active and unencumbered in Tennessee. Substantial chronic care management experience may substitute for RN licensure.
Experience: Three or more years in care management, care coordination, or ambulatory nursing, including at least one year supervising or precepting clinical staff. Direct CCM or APCM experience strongly preferred.
What success looks like in the first 6 to 12 months
- Engagement rate at or above 60 percent
- Billable patient rate at or above 60 percent
- Patient satisfaction at or above 9 of 10
- Every care manager receiving documented monthly review and coaching
- Documentation quality consistent enough to withstand an external audit without remediation
