Job Type: 3+ Month Contract (Potential for Extension)
Pay Rate: $35.86/hour (W-2)
Schedule: Monday-Friday | 8:00 AM - 5:00 PM CST
Clinical Role
We are seeking a Registered Nurse (RN) with an active, unrestricted Illinois RN license for a Field-Based Nurse Case Manager position.
This role is ideal for experienced RNs who are passionate about providing case management services to elderly individuals and people with disabilities receiving in-home or nursing facility care.
Location Requirements
Candidates must currently reside in Illinois, specifically in one of the following counties: Mileage reimbursement is provided for eligible travel.

- DuPage County
- Will County
- Lake County

Key Responsibilities
- Conduct comprehensive member assessments using clinical tools, medical records, and available data to determine member needs, eligibility, and appropriate interventions.
- Develop, implement, and monitor individualized case management plans to improve health outcomes and ensure cost-effective care.
- Coordinate care by collaborating with healthcare providers, caregivers, and community resources.
- Apply clinical guidelines, case management protocols, regulatory standards, and company policies to ensure appropriate administration of benefits.
- Advocate for members by identifying appropriate internal and external resources to address healthcare needs.
- Maintain accurate and timely documentation while complying with regulatory, accreditation, and company requirements.
- Utilize case management and quality management processes to support optimal member outcomes.
- Work independently while effectively managing a field-based caseload and maintaining strong communication with the virtual care team.
Required Qualifications
- Active, unrestricted Illinois RN License
- Previous Case Management experience
- Ability to travel 50–75% within the assigned territory
- Strong organizational, communication, and independent decision-making skills
- Reliable transportation for field visits
