Responsibilities
- Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations to determine individualized care plans.
- Deliver skilled nursing interventions such as wound care, medication management, IV therapy, and infectious disease care within the home setting.
- Collaborate with physicians, case managers, and other healthcare professionals to develop and adjust patient treatment strategies based on ongoing evaluations.
- Educate patients and their families on health management, discharge planning, medication adherence, and safety protocols to promote independence.
- Maintain accurate and detailed medical records in compliance with HIPAA regulations and hospital standards.
- Perform diagnostic evaluations and coordinate outpatient or post-acute care services as needed to ensure seamless patient transitions across care settings.
- Manage patient intake processes and facilitate utilization management by prioritizing urgent cases and optimizing resource allocation.
Qualifications
- Valid RN license with current state certification; additional certifications in critical care, ICU, trauma center experience or emergency medicine are highly desirable.
- Demonstrated experience in hospital medicine, medical-surgical units, or acute care environments such as Level I or Level II Trauma Centers.
- Proven expertise in managing complex cases involving geriatrics, behavioral health, ICU, or post-acute care settings.
- Strong knowledge of medical terminology, physiology, infection control practices, and discharge planning procedures.
- Experience with hospital records management, diagnostic evaluation techniques, case management processes, and outpatient clinic workflows.
- Ability to perform patient assessment accurately while maintaining HIPAA compliance and safeguarding sensitive medical records.
- Excellent communication skills for patient service delivery and interdisciplinary collaboration; proficiency in intake procedures and utilization management is a plus.
