Responsibilities:
Provide direct patient care, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the healthcare team
Monitor and report changes in patient condition to the nursing staff and physicians
Participate in discharge planning and education for patients and families
Maintain accurate and thorough patient records
Collaborate with interdisciplinary team members to ensure seamless care transitions
Perform other duties as assigned by the nursing staff and management

Requirements:
Currently licensed as a Licensed Practical/Nurse (LPN) in the state of Wisconsin
High school diploma or equivalent required
1-2 years of experience as an LPN preferred
