Home Health Scheduler / Care Coordinator (Remote)Full-Time | Monday–Friday | 8:00 AM–5:00 PM CST
Must have previous experience using WellSky (Kinnser). This is a non-negotiable requirement.
Outcomes Therapy is a therapist-owned home health company serving Central Texas. We're looking for an experienced Home Health Scheduler/Care Coordinator who can hit the ground running. This is not an entry-level position—we need someone who already understands home health workflows and is comfortable working in WellSky (Kinnser) from day one.
Responsibilities
- Schedule home health PT, OT, and ST visits
- Manage incoming referrals and coordinate admissions
- Assign clinicians based on coverage and availability
- Communicate with therapists, referral sources, agencies, patients, and families
- Maintain accurate documentation and patient records in WellSky
- Monitor referral status and ensure timely follow-up
- Help keep schedules organized and patients seen on time
Requirements
- Required: Previous experience using WellSky (Kinnser).
- Required: Home health scheduling or care coordination experience.
- Strong organizational and multitasking skills
- Excellent communication and customer service skills
- Ability to work independently in a fast-paced remote environment
- Knowledge of insurance verification and home health processes is a plus

Pay: $18.00 - $24.00 per hour
