Staff Social Workers - Nursing Home
Located in Brooklyn, NY
Salary Range - $75K to $90K Per Year (based on experience/facility)
Long term care facilities located in Brooklyn, NY are seeking strong Staff Social Workers.
Responsibilities:
- Plans and directs a comprehensive social services program which provides support services for residents and families.
- Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
- Participates in an interdisciplinary team approach to better serve the needs of the facility population.
Qualifications/ Requirements:
- Must have BSW, MSW or LMSW
- Must have minimum 2 years of experience
- Must have prior case management experience
- Must have long term care experience
- Ability to communicate effectively
- Strong written and verbal communication skills
