Job Summary:
At Healing Headquarters Inc/Precision Wound Care of Florida, we are seeking a compassionate and experienced Nurse Practitioner (NP) to join our PRN team serving Alabama or the Panhandle of Florida. The ideal candidate will deliver an integrated mix of in-home wound care, primary care, and functional medicine, with the ability to provide telemedicine visits when appropriate. You will care for patients across settings, including homes, assisted living facilities, and skilled nursing centers, using a patient-centered, evidence-based approach to improve outcomes, reduce avoidable utilization, and support long-term wellness.
Key Responsibilities:
- Perform comprehensive wound assessments and develop evidence-based treatment plans
- Manage pressure injuries, diabetic foot ulcers, venous and arterial ulcers, surgical wounds, and other complex skin integrity conditions
- Conduct routine follow-up evaluations and monitor wound healing progression
- Utilize advanced wound care modalities and best-practice treatment protocols
- Provide clinical documentation that meets regulatory and reimbursement standards
- Deliver high-quality care while helping reduce avoidable hospitalizations and wound-related complications Provide primary care assessment, diagnosis, and treatment for common acute and chronic conditions, including medication management and care plan development.
- Incorporate functional medicine principles as appropriate (e.g., nutrition, lifestyle, sleep, stress, and root-cause assessment) into individualized care plans and patient education.
- Collaborate with physicians, home health, facility staff, specialists, and community resources to optimize outcomes and close care gaps.
- Conduct telemedicine visits (video/phone) for appropriate follow-ups, urgent questions, medication review, chronic disease check-ins, lab review, and care plan updates; determine when an in-person visit is necessary based on clinical findings and risk.
- Triage incoming patient concerns via phone/video, provide evidence-based guidance, and escalate to same-day in-person assessment, ED referral, or specialist consultation when clinically indicated.
- Provide a high-quality virtual patient experience by setting expectations for the visit, confirming connectivity, communicating clearly, and documenting patient questions; collaborating with staff to resolve basic technology barriers.
- Educate patients and caregivers on wound care, chronic disease self-management, and lifestyle interventions (e.g., nutrition, activity, smoking cessation) to support healing and long-term health.
Qualifications & Requirements:
- Active Nurse Practitioner (NP) license in Alabama (or ability to obtain prior to start).
- Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care NP (AGPCNP) preferred.
- Minimum 2 years of clinical experience in wound care, primary care, telehealth/telemedicine, or a related ambulatory care setting.
- Working knowledge of chronic disease management, preventive care, and telemedicine workflows; interest or training in functional medicine is strongly preferred.
- Excellent communication and interpersonal skills for patient-centered care.
- Ability to work independently in a mobile setting and deliver high-quality care via in-person visits and telemedicine.
- Valid driver's license and reliable transportation for travel throughout Mobile and Baldwin Counties.
