Patient Outreach Healthcare, Inc. is a Medicare-certified home health agency proudly serving patients throughout Lehigh, Northampton, Berks, Bucks, Montgomery, Carbon, and Philadelphia Counties. We are currently seeking a compassionate, motivated, and skilled Occupational Therapist (PRN) to join our growing clinical team.
As a Home Health Occupational Therapist, you will help patients regain independence, improve safety in the home, and maximize their ability to perform daily activities while delivering high-quality, patient-centered care.
Responsibilities:
- Perform comprehensive occupational therapy evaluations in patients' homes.
- Develop individualized treatment plans based on physician orders and patient needs.
- Provide skilled occupational therapy interventions to improve functional independence.
- Assess home safety and recommend adaptive equipment and environmental modifications.
- Educate patients, families, and caregivers on therapeutic techniques and home safety.
- Collaborate with physicians, nurses, physical therapists, speech therapists, and other members of the interdisciplinary team.
- Complete timely, accurate, and compliant clinical documentation.
- Participate in care conferences and communicate changes in patient condition.
Qualifications
- Current Pennsylvania Occupational Therapist (OT) license in good standing.
- Graduate of an accredited Occupational Therapy program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and automobile insurance.
- Excellent communication, organizational, and documentation skills.
- Previous home health experience is preferred but not required.
