Duration: 13 weeks
Start Date: 2–3 weeks from job offer
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Hours: 40 hours per week

Mileage Reimbursement: $0.72 per mile
The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimal utilization of resources, effective service delivery, and compliance with prescribed medical regimens.
Key responsibilities include:

Coordinating and managing patient care plans
-
Collaborating with interdisciplinary healthcare teams
-
Monitoring patient progress and documenting care

Providing education and support to patients and families
-
Ensuring compliance with agency, state, and federal regulations
-
Current RN license in Massachusetts
-
Minimum 2 years of hospice and/or home health experience
-
At least one year of case management experience preferred

Current CPR certification (if applicable)
-
TB questionnaire, PPD, or chest X-ray (if applicable)

Current health certificate (per contract or state regulations)

Must meet all federal, state, and local requirements

Must be at least 18 years of age
