Job Summary
We are seeking a dedicated and compassionate Home Care Nurse to provide exceptional nursing care to patients in their homes. This vital role involves delivering personalized, high-quality healthcare services, promoting patient independence, and ensuring safety and comfort throughout the care process. Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and review of medical history to determine care needs.
- Administer medications, treatments, and injections following prescribed protocols while monitoring for adverse reactions.
- Provide wound care, IV therapy, and other specialized procedures with precision and attention to infection control standards.
- Educate patients and their families on disease management, medication usage, lifestyle modifications, and post-discharge care plans.
- Coordinate discharge planning and facilitate communication with hospital teams for smooth transitions of care from inpatient to outpatient settings.
- Document all patient interactions accurately in medical records while adhering to HIPAA regulations to ensure confidentiality.
- Collaborate with interdisciplinary teams to develop and adjust individualized care plans based on ongoing assessments and patient progress.
Skills
- Extensive background in hospital medicine, emergency medicine, or post-acute care environments including outpatient clinics or home health services.
- Knowledge of physiology, medical terminology, diagnostic evaluation techniques, and utilization management processes.
- Exceptional patient service skills with the ability to perform thorough patient assessments and deliver compassionate care.
- Strong organizational skills for intake procedures, discharge planning, case management, and medical records documentation.
- Ability to adapt quickly in high-pressure situations such as emergency room scenarios or behavioral health crises while maintaining professionalism.
